Each Excel sheet (tab) is one well. The survey columns:

Well name. Comes from the name column if present; otherwise from the sheet name. The name column accepts variations (case/accent-insensitive): Well_Name, Well Name, Nome do Poço, Poço, Nome.
Filling the name on a single row is enough (no need to repeat it on every row) — or leave the column out and name the sheet after the well. Sheets without an MD column are ignored.
